Tully RX

Telehealth Informed Consent

Last updated: [DATE — set at counsel sign-off]

What it means to receive care through Tully RX — how telehealth and asynchronous review work, what they can and can't do, and what you're agreeing to.

01Nature of telehealth services

Telehealth means receiving healthcare services remotely, using electronic communication instead of an in-person visit. Through Tully RX, licensed healthcare providers review your health information and, where clinically appropriate, diagnose, treat, and prescribe.

Telehealth has benefits (convenience, access) and limitations: your provider cannot perform a hands-on physical examination and relies on the information you provide. Some conditions cannot be safely treated via telehealth, and your provider may direct you to in-person care.

02Asynchronous (store-and-forward) care

Your care may be asynchronous: you complete a structured health assessment, and a licensed provider reviews it later — without a live video or phone visit — before making any treatment decision. Your provider may follow up with questions through the portal, and you can message your care team at any time.

The quality of asynchronous care depends on the accuracy and completeness of your answers. You agree to answer truthfully and to update your care team if your health changes.

03Prescription policy

  • Whether to prescribe is always the treating provider's independent clinical decision. No prescription is guaranteed.
  • You are charged only if a provider approves treatment; the payment hold placed at checkout is released if they do not.
  • Prescriptions are filled and shipped by licensed pharmacies, which may include compounding pharmacies preparing customized medication.
  • Placeholder for counsel: controlled-substance policy statement and any product-specific (e.g. compounded medication) disclosures.

04Emergencies

Tully RX is not an emergency service and is not monitored in real time. If you are experiencing a medical emergency — including chest pain, difficulty breathing, signs of a severe allergic reaction, or thoughts of harming yourself or others — call 911 or go to the nearest emergency room immediately. If you are in crisis, you can also call or text 988 (Suicide & Crisis Lifeline).

05State licensure and availability

Care is provided only by providers licensed in the state where you are located at the time of service, and only in states where the service is available. You agree to accurately report your location; providing a false location can invalidate your care.

Placeholder for counsel: list of covered states and any state-specific consent language (several states require additional telehealth disclosures).

06Privacy and health information

Your health information is handled under HIPAA-grade safeguards: stored encrypted, shared only with your care team, the reviewing provider, and the fulfilling pharmacy, and never sold. Electronic communication carries inherent risks despite safeguards; we mitigate these with encryption and access controls.

See the Privacy Policy for full details on collection, use, and your rights.

07Your rights

  • You may withhold or withdraw this consent at any time, without affecting your right to future care, by stopping use of the service and notifying support.
  • You may request a copy of your medical records.
  • You may ask your provider questions about your treatment and any alternatives, including in-person options.

08Consent acknowledgement

By checking the consent box during your assessment (or by proceeding with treatment), you confirm that:

  • You have read and understood this consent, and had the chance to ask questions.
  • You understand the nature and limitations of telehealth and asynchronous care.
  • You understand that no prescription is guaranteed and that treatment decisions rest with a licensed provider.
  • You are the person receiving care, you are physically located in the state you reported, and the information you provide is accurate.

Placeholder for counsel: exact acknowledgement wording, versioning, and how/where the timestamped consent record is captured.
